Cost and Insurance

We accept private pay clients that are in-network with the following insurance plans:

• Regence BlueCross BlueShield

• Moda Health

• Aetna

• Providence

• Moda

• PacificSource

If your insurance is not listed above, we are not in-network. We can provide a superbill/invoice for you to submit to insurance for out-of-network reimbursement.

Private pay rates are available upon request for those without insurance, those who do not want to use their in-network plan or those with out-of-network insurance plans. Please email info@cmbh.health to inquire about private pay rates.

Forms and Paperwork

All forms and letters must be completed during a separate scheduled appointment.

No Show and Late Cancellations

All appointments must be cancelled within 2 business days prior to the scheduled visit. No shows and late cancellations will be charged $150.00 for the missed visit.
